→ What is the CMS Guide Model?
On July 31, 2023, the Centers for Medicare & Medicaid Services (CMS) announced a new voluntary nationwide model – the Guiding an Improved Dementia Experience (GUIDE) Model – a model test that aims to support people living with dementia and their unpaid caregivers. The GUIDE Model will focus on dementia care management and aims to improve quality of life for people living with dementia, reduce strain on their unpaid caregivers, and enable people living with dementia to remain in their homes and communities. It will achieve these goals through a comprehensive package of care coordination and care management, caregiver education and support, and respite services.
→ How can Be Light Care Consulting help GUIDE model participants meet CMS care delivery requirements?
Partner Organizations are considered contracted providers that can help GUIDE care participants meet the rigorous delivery requirements required by CMS, either in person or virtually. One of the many delivery requirements required by CMS is "Caregiver Education and Training" for which Be Light Care Consulting is expertly able to provide.
→ How much will it cost to partner with Be Light Care Consulting as our "Caregiver Education and Training" provider?
Each contract will vary depending on the services selected by the participant as well as the contract length. Discounts will be available for multi-year contracts. Services may include: mobile app video access for beneficiaries, mobile app brand whitelisting, online course access, digital and printable product access and exclusive content (live Q&As, etc.).
